P-009 Notification of Death or Serious Injury

PURPOSE: 

To establish a policy which is to be followed should a Halifax Regional Fire & Emergency member, while performing fire service business, suffer serious injury or injuries which prove to be fatal either at the scene or later in the hospital.

OBJECTIVE:

The objective of this policy is to ensure all HRFE members are aware of the process used during notification upon death or serious injury of a member. 

SCOPE:

This policy applies to career and volunteer members of HRFE. 

POLICY:

  • DEATH AT SCENE:

The Chief Director, Deputy Chief Director, Divisional Captain of Safety, Manager of Corporate Safety, Fire Service Chaplain, FFAP Coordinator, and the appropriate Platoon Chief, Fire Chief or Divisional Chief shall be notified immediately. The Chief Director or his/her Designate, the Fire Service Chaplain and a Union representative or a close friend within the Fire Service will notify the member’s family of his/her death. The FFAP Coordinator will check the LODD Form FD-105.

  • INJURY WITH DEATH IMMINENT:

The Chief Director, Deputy Chief Director, Divisional Captain of Safety, Manager of Corporate Safety, Fire Service Chaplain, FFAP Coordinator, and the appropriate Platoon Chief, Fire Chief or Divisional Chief will be notified immediately. The Chief Director or his/her Designate, the Fire Service Chaplain will then go, in person to the member’s home, to advise the family of what has happened and escort them to the hospital. The FFAP Coordinator will check the LODD Form FD-105.

NOTE:

The Divisional Captain of Safety or the Manager of Corporate Safety will ensure that notification is made to the Nova Scotia Department of Labour and Workforce Development Occupational Health & Safety Division.
